Aviva Health will continue to grow as a recognized, respected and integral part of the healthcare delivery system in Douglas County, providing basic care and access to services that meet the needs of our patients.

Aviva Health values universal access to appropriate healthcare, the inherent dignity of all people and partnerships and collaborative relationships that develop resources to meet the needs of our community. In order to fulfill our mission, vision and values, every employee at Aviva Health is expected to take initiative and follow through with each interaction in a manner that helps to assure that every customer, either external or internal, experiences extraordinary service.

What is an FQHC?

Aviva Health is a Federally-Qualified Health Center,  or FQHC. We are a non-profit healthcare practice that provides high-quality, cost-effective primary healthcare to anyone seeking care, regardless of their insurance status or ability to pay. A discounted sliding fee schedule is available to patients based on family size and income.

FQHCs are:

  • Affordable and Open to Everyone. Fees are based on income and family size, whether or not you are insured.
  • Patient-centered, Community-centered.    
    Aviva Health is recognized by the State of Oregon as a Patient-centered Primary Care Home. Watch this video to learn more.
  • Governed by Patients. At least 51 percent of the board members of an FQHC must be consumers of the health center’s services, ensuring patient and community involvement in service delivery.
  • Improve Outcomes and Reduce Disparities. Community Health Centers are instrumental in reducing health disparities and improving outcomes for persons with chronic illness through care management.

Aviva Health began seeing patients in 1991 as the Open Door Clinic, the effort of a community task force intent on providing health care for the underserved. Located on Pine Street, the clinic provided 1,241 visits that first year. Aviva Health became an FQHC in 1999 and has grown to six service locations, providing primary medical, dental and behavioral health services. In 2016, Aviva Health provided over 45,000 patient visits.

Aviva Health is an FTCA-deemed facility. The Federally Supported Health Centers Assistance Act of 1992 and 1995 granted medical malpractice liability protection through the Federal Tort Claims Act (FTCA) to HRSA-supported health centers. Additionally, Aviva Health receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
